Overview

The Heart Chart app is a blood pressure and health metrics tracking app designed by doctors to help users collect and manage their blood pressure readings along with other health metrics. The app is created to help users measure their blood pressure accurately using their own home monitor, follow their trends over time, and help their healthcare provider best manage their care. The app provides a "how-to" measurement guide to ensure accurate blood pressure measurements and a Swipe Averaging™ feature to rapidly and efficiently calculate blood pressure averages. The app also offers information on blood pressure from internationally renowned experts.

The Heart Chart app supports Bluetooth-enabled blood pressure monitors, including the Equate 8500 Series Premium Upper Arm Blood Pressure Monitor, Equate 6000 Series Upper Arm Blood Pressure Monitor, and Equate 6500 Series Wrist Blood Pressure Monitor. However, the app is not intended for the diagnosis or screening of hypertension. Users should be aware that the app is an information management service to enable the analysis of blood pressure data and is not a substitute for the advice of a healthcare professional. Users should always consult their doctor or other qualified healthcare professional with any questions they may have regarding a medical condition, including any queries or concerns about blood pressure management.
